Individual Rate: $95

Employer Rate for Employees: $85 each (must have 5 or more members to qualify)

Family Rates:

• 2 adults +1 child: $225
• 2 adults + 2 children: $250
• 2 adults +3 or more children: $300
• 1 adult +1 child: $150
• 1 adult + 2 more children: $200

Couple Rate: $175

Enrollment fees applied.

New psychiatric Evaluation: $275

Follow up psych med management: $150

Vit B12 shots: $30 per shot

ADHD Evaluation: $500 with creyos including administration and interpretation of results.  Additional $150 for comprehensive narrative report.

Neurostar TMS: $350 per treatment, 1 package discount applies. Covered by most insurance.

Psychotherapy: $150 per session

Medical Visit: $150 does not include labs

Yellow Fever Vaccine: $250

Pregnancy Test: $$25

UA to Check for UTI: $35

Drug Screen: $70

Wellness panels include CBC, CMP, Lipid Panel ,HA1C
